Introduction: Perimortem caesarean section (PMCS) is a resuscitative measure carried out on a pregnant woman when there is failure of return of spontaneous circulation (ROSC) during cardio-pulmonary resuscitation (CPR). When performed promptly, it has been found to improve maternal and foetal outcome. However, due to rarity of maternal cardiac arrest, most obstetric units are often unprepared when faced with this emergency, thus leading to catastrophic maternal and foetal outcome.
Case report: A 33-year-old multipara who presented to the delivery suite at term with labour pains. She had no other complaints on initial review by the midwives. Patient was noted to have had a sudden projectile vomiting and subsequently became unresponsive. CPR was immediately instituted. PMCS was thought of nine minutes after failure of ROSC and performed 5 minutes later due to the non-availability of scalpel within the delivery unit. A female neonate with severe birth asphyxia with normal weight was delivered. Maternal resuscitation was continued, however, patient was certified dead after 11 minutes. Autopsy was declined by the relatives. Baby died after 67 days in special care baby unit.
Obstetricians faced with this often encounter delays in deciding when to perform the procedure in addition to the challenges of availability of resources to do so. There is need for awareness and drills on this life-saving procedure.
