Introduction: Acute diverticulitis is a frequent reason for surgical admissions, particularly in older patients. NICE guidance updated in 2023 recommends avoiding antibiotics in patients with uncomplicated disease who are systemically well, to reduce unnecessary treatment and hospital stays.
Aim: This re-audit looked at how our trust is following these guidelines and whether there has been improvement since the first audit cycle.
Methods: We reviewed notes for patients presenting with acute diverticulitis at two district general hospitals between June and December 2024. Patients who were immunocompromised, significantly comorbid or systemically unwell were excluded. Data collected included antibiotic use, vitals, inflammatory markers and whether patients were managed through ambulatory care. Results were compared with the first audit cycle.
Results: Of 81 patients identified, 54 met inclusion criteria. Mean age was 58 (range 25–89), with 63% female. 81.4% of patients were treated with antibiotics and 18.6% were managed without, which was an improvement compared to 5.7% previously. Use of ambulatory care remained very low (3.7%). No patients required ITU or radiological intervention.
Conclusion: There has been some improvement in following non-antibiotic guidance but rates are still well below the 95% target. Ambulatory care uptake also needs work. Clearer guidance on terminology and more visible reminders of the NICE pathway may help.
