Introduction: Stoma formation presents not only physical and psychological challenges but also significant financial burdens for patients. Socioeconomic disparities (SEDs), especially in the context of increasing global migration, play a crucial role in shaping experiences and outcomes for colorectal patients with stomas.
Aim: This scoping review aims to explore the influence of SEDs at the experiences of cancer ostomates. It seeks to identify gaps and better understand the mechanisms through which these disparities impact care to help adjust interventions and policy towards equitable colorectal cancer care delivery.
Methods: A systematic search of Medline and Embase was conducted by two independent reviewers, identifying eight peer-reviewed articles published over 20 years. English-language studies were assessed and selected using predefined eligibility criteria within the Arksey and O’Malley framework, following the PRISMA guidance.
Results: There is evidence of awareness and acknowledgment of how SEDs have influences of ostomates post operatively. Though few studies directly addressed SEDs, findings suggest that rural residency is linked to lower quality of life (QoL) scores compared to urban counterparts. Employment was often negatively affected, with job loss reported in some cases. Financial strain was notably higher among men, even those with insurance. However, no significant association was found between QoL and factors such as religion or education.
Conclusion: A deeper understanding of SEDs in colorectal care is essential. Addressing these disparities can inform surgical planning, enhance pre-operative counselling, and improve post-operative support through equitable policies, such as better pricing, reimbursement, and centralised access to stoma supplies.
