Postoperative Pain Management In Urological Surgeries
Ruben Raj Ellan Govan TAYSIDE, Dundee, United Kingdom
Introduction: Effective postoperative pain control is essential in urological surgery, particularly in settings where resources and staffing may limit analgesic review. Initial observations at NHS Tayside revealed variable prescribing practices and inconsistent pain outcomes among both cancer and benign urology patients.
Aim: To evaluate and improve postoperative analgesic practices in urological surgery, focusing on non-steroidal anti-inflammatory drug (NSAID) use, opioid minimisation, and early pain control.
Methods: A three-cycle quality improvement project was conducted involving 120 patients undergoing minor and major urological procedures. The first cycle identified suboptimal NSAID and opioid prescribing. An analgesia protocol flowchart was co-developed with the pain and urology teams and disseminated to junior doctors. A second cycle assessed changes following implementation. Subsequently, nursing staff received formal education before a third re-audit cycle. Ethical approval was obtained through local clinical governance.
Results: Postoperative pain management improved markedly across cycles. NSAID use increased from 28% to 42% and finally to 65%, while codeine prescribing at discharge decreased from 70% to 58% and then to 38%. Actual opioid use fell from 40% to 35% and finally to 25%. Mean pain scores at 24 hours postoperatively decreased from 6.8 to 6.1 and then to 4.9, reflecting enhanced adherence to multimodal analgesia and improved patient outcomes.
Conclusion: Low-resource, education-driven interventions can sustainably improve postoperative pain management. This model is transferable to resource-limited surgical settings globally.
